Enter the order number of the order you purchased using Partial.ly. You can get the order number from the order confirmation email sent when you originally checked out. You can also see your order number within your Partial.ly Dashboard by going to the Dashboard section in the side menu and clicking the 'Other Payment Plans' button. Click 'View Details' next to your Wild Roots payment plan and you will see your order number labelled as 'Merchant Order ID'.

Email address(Required)

Enter the email address you gave us when you originally checked out. This is the same email address that we sent your order confirmation email to. We will use this email address to communicate with you about your request.

Partial.ly email address(Required)

Enter the email address that was used to create your Partial.ly account, even if it is the same as the above email address.
